Yes exactly, you don't have to worry about it. I would reccomend the msvc 2008 
variant as you most likely have those runtimes already installed.

No but the mapserver packages usually contain their own mapscript versions and 
you need at least a version > 6. 

Just to clarify:
If you install Mxd2map from the binary windows executable you don't need a 
mapserver to convert mxd files the installer already contains everything it 
needs to do this (except arcgis, of course). The mapserver is then needed to 
draw the map mxd2map creates. So you don't need to worry about mapscript 
versions that much. Thats only necessary if you want to install mxd2map 
directly from source without using the installer package.
